What to do before your first appointment:

Before coming to your appointment, please make sure to create a patient/client profile online, if you a have not done so already.  Please complete your intake form for the practitioner you are seeing. If you see multiple practitioners here at Sustainability Wellness, please make sure you complete the forms for each practitioner. Some practitioners require different forms, or more detailed information. If you are unable to complete forms prior to your session, please let us know prior to your appointment and arrive 15-20 minutes prior to your session to complete the forms with our front desk personnel.



Sustainability Wellness wants to ensure the safety of those whom we treat and our staff. Due to the nature of our “hands-on” facility, if you are sick or start to feel the symptoms of being sick, we ask that you cancel and reschedule your appointment when you are feeling better, without penalty. If you appear to be ill at the time of your session, the practitioner may cancel your appointment, and reschedule the session, without penalty. We thank you for your help and understanding to keep Sustainability Wellness a safe and healing environment.



Patient/client will notify Sustainability Wellness, or their practitioner, if they are running late for their session. If the patient/client is late, the practitioner reserves the right to continue on with the treatment but the treatment will still end at the time it was scheduled to be over and the price of the treatment remains the same to what was scheduled. When canceling an appointment, it is imperative that the patient/client cancels 24 hours prior to the scheduled appointment. The client will be charged the full session amount if he/she cancels under 24 hours. If a patient/client fails to show for their scheduled appointment, the patient/client will be charged the full price of the session. 

Patients/clients that are using medical insurance will be charged the cash rate for no show and canceled appointments under 24 hours. It is the responsibility of the patient/client to pay for the missed/canceled appointment. Insurance can only be applied to services rendered.
